Doctor-Patient Relationship

Accessing our websites, submitting a contact form, or communicating with the Practice through a website does not by itself establish a doctor-patient relationship.

A doctor-patient relationship is established through the Practice’s normal patient intake and treatment process.

Website Communications

Website forms are intended for general questions, appointment requests, and other appropriate communications.

Please do not use website forms to communicate medical emergencies or unnecessarily detailed confidential medical information.

Accuracy of Information

We make reasonable efforts to provide accurate and current information but do not guarantee that all website content is complete, accurate, or current at all times.

The Practice’s privacy practices are designed to address applicable HIPAA requirements concerning the use and disclosure of PHI, patient privacy rights, workforce responsibilities, and safeguards for protected health information.

The Practice does not sell patients’ protected health information.

Where applicable, the Practice limits access to protected health information to authorized personnel who have a legitimate need to access the information.

The Practice maintains administrative, physical, and technical safeguards designed to protect electronic protected health information against unauthorized access, use, disclosure, alteration, or destruction.

The Practice also maintains procedures addressing the identification and response to potential privacy or security incidents and breaches as required by applicable law.

The Practice follows policies designed to limit uses, disclosures, and requests for protected health information to the minimum information reasonably necessary to accomplish the intended purpose, when the HIPAA minimum necessary standard applies.

HIPAA Security

The Practice uses reasonable and appropriate administrative, physical, and technical safeguards designed to protect electronic protected health information.

Website communications use HTTPS, and information submitted through website forms is transmitted through secured email.

Website form information is stored in Logan, Utah, for up to 90 days and access is restricted to authorized Practice personnel.

The Practice does not use third-party vendors to receive or store information submitted through these website forms.

Notice of Privacy Practices

The Practice maintains a separate Notice of Privacy Practices explaining how protected health information may be used and disclosed and describing patients’ privacy rights.
